Problems in Pregnancy and its Treatment

♦Nausea & Vomiting feeling during pregnancy

  • Nausea & Vomiting feeling occurs specially in morning also called morning sickness, more in first pregnancy, usually appear by the end of first or second month and improve by the end of first trimester, 50% women have both nausea and vomiting, 25% have nausea and 25% are unaffected !

Treatment of nausea and vomiting during pregnancy

  • Dietary changes : take dry toast, biscuits and protein rich meals, frequent small foods, avoid fatty food
  • Behavior modification: to avoid personal triggering factors.

♦Why does backache occur in pregnancy?

Common problem, around 50% females have it.

Cause:

  • Physiological changes – Joint ligament laxity, weight gain, hyperlordosis and anterior tilt of pelvis
  • Others – faulty posture, High heel shoes, muscular spasm, urinary infection, constipation
  • Treatment of backache during pregnancy-
  • Avoid excessive weight gain
  • Rest with elevation of legs
  • Improvement in posture
  • Comfortable footwear
  • Well fitted pelvic girdle belt
  • Rest in hard bed
  • Massaging the back muscles, analgesics and rest relieve the pain due to MS spasm.

Constipation in pregnancy and its treatment-

Quite common

Cause

  • Decreased tone of gut due to effect of hormone progesterone
  • Diminished physical activity
  • Pressure of pregnant uterus on the pelvic colon (intestine)

 

Treatment of constipation during pregnancy-

  • Fiber rich diet
  • Regular walk (If not medically contraindicated)
  • Plenty of fluids
  • Use of laxative(Medicine) as prescribed by doctor, if no relief.

♦Leg Cramps during pregnancy and its cure-

  • May be due to deficiency of diffusible serum calcium or elevation of serum phosphorous.

 

Steps of follow to avoid leg cramps

  • Supplementary calcium therapy as tablet or syrup after principal meals helps.
  • Massaging the leg
  • Application of local heat and Intake of tablets having vitamin B1(Thiamine) on regular basis also helps.

  • Peas
  • Foods fortified with vitamin B1 available in market: rice, pasta, breads, cereals & flour(check the label to confirm fortification)

 

Acidity and heartburn common problem during pregnancy

  • Common in pregnancy due to esophageal sphincter relaxation

Treatment of acidity and heartburn during pregnancy-

  • Avoid over eating
  • Avoid going to bed Immediately after meals
  • Sleep in semi reclining position with high pillows relieves symptoms
  • Liquid antacids may help

 

Varicose Veins in pregnancy

  • Swelled up / engorged veins visible in legs and vulva or rectum (haemorrhoids/ piles)
  • May appear for the first time or aggravate during pregnancy, usually in the later months
  • Cause: due to obstruction of blood flow returning to heart via veins by the pregnant uterus

Treatment of varicose veins:                                                     

  • Tying elastic crepe bandage while movement of limbs
  • Elevation of legs during rest

Vulva varicose veins:

Vulvar veins are varicose {blood vessels |leg veins} at the external surface of the female genitalia (vulva). They happen regularly during pregnancy. This is due to expansion in blood vessels.

 

What measures should be taken if Vulva varicose veins appears during pregnancy?

  • Usually disappear after delivery
  • If severe try to limit prolonged movements and standing
  • Take frequent rests in between
  • While taking rest keep a pillow beneath the hips to elevate the pelvis and help the veins drain via gravity. Also such a position shifts the pregnant uterus backward and hence relieves the pressure and obstruction
  • Avoid putting on excessive weight!

Hemorrhoids:

  • Hemorrhoids are swollen veins in the anus and rectum region  which are common during pregnancy most common during the last three months of pregnancy
  • May cause annoying complication like bleeding or may get prolapsed
  • Treatment regular use of laxative as prescribed by doctor
  • Local application of hydrocortisone ointment
  • Surgical treatment should better be withheld since condition improves remarkably after pregnancy

 Please Note-Specific treatment should better be avoided for varicosities, varicose veins in leg and Hemorrhoids since most of the times these conditions disappear after delivery!

Swelling in Ankle during Pregnancy-

  • More by evening, as the day progresses and relieved on waking up after overnight’s rest
  • This is physiological or orthostatic edema. No treatment is needed for this
  • One should get herself checked for any ankle edema with a doctor so that serious conditions like preeclampsia can be looked for

Vaginal Discharge during Pregnancy-

  • There is increased vaginal discharge during pregnancy due to increased blood flow to the reproductive organs and increased level of hormone estrogen during pregnancy.
  • The discharge is mucoid and non-irritating
  • No treatment is required
  • Women is only advised to maintain personal hygiene
  • If the doctor feels the need to examine the discharge, and the microscopic examination reveals presence of any infection (Like trichomonas, candida, bacterial vaginosis) one would be prescribed ointments for local application.
